Device Licensing

LICENSE REQUIREMENTS

An ADWM license is required for any commercial weighing, measuring or counting device used for commercial purposes in Arizona. If you are obtaining a license in the name of an individual, A.R.S. § 41-1080 requires you to fill out a form and submit proof of citizenship. NEW DEVICES 

An ADWM device license must be obtained within the first 30 days of commercial use of the device (R-20-2-201). To obtain a device license, just send a completed Placed In Service Report (PISR). You will be issued a Business Multi-Function (BMF) number and billed for the device licenses once the PISR is reviewed and approved. Your BMF number is your Agency ID number.

Licensed device changes must be reported to the Department within 7 days of the change. If transferring a license, the new licensee shall notify the Department of the following: new business name, address and the location of the device(s). NOTE: all devices used for commercial purposes must be NTEP-approved.

RSA Information


A Registered Service Agency (RSA) usually installs and calibrates commercial devices. Click here to view a list of RSA's. The RSA is licensed by the Department and will submit the completed license application on behalf of your business.

PLACED IN SERVICE REPORT DIRECTIONS

1. Complete the business name and location information. Please indicate the BMF number if one has been previously issued.

2. Complete the Owner/Licensee information. If the Owner/Licensee is an INDIVIDUAL a Licensing Eligibility Form MUST be completed and submitted. This form is available online at www.azdwm.gov Click on “Forms” then on “Licensing Eligibility Requirements.”

3. Complete the business billing information. If the information is the same as the billing information, you can indicate "same as above."

4. If a RSA is used, indicate the RSA name and the RSR (technician) and license number.

5. Use a separate line to indicate the following information for each device:

  • FEE CODE
  • SERVICE CODE AND SERIAL NUMBER FOR EACH DEVICE
  • NTEP NUMBERS (REQUIRED FOR ALL DEVICES INSTALLED AFTER January 1, 1975).
  • A TAG NUMBER (REQUIRED FOR ALL TAGGED DEVICES BEING REPAIRED). (Indicators should be shown as a separate device since they have a unique serial number.)

6. The RSR must indicate the serial numbers of equipment used to install or repair the device. This equipment must have a current certification.

7. Both RSR and Business Representative must sign and date the form.

8. Additional forms can be used when there are more than 10 devices. Indicate the number of total pages in the lower right of the form.

9. Completed forms must be sent to the Department within 7 days of the service date.

10. When an RSR is replacing a device, the serial number of the obsolete device must be noted. Use service code "O" for devices that have been replaced.
