In order to process the application in a timely manor please review it and make sure that all the requested information is complete and accurate. If it is not complete this could result in a denial of the application and returning it to you for completion. Make sure that all the requested attachments (Site Plan, Materials List, and Scope of Work), are included. Also make sure that the application is legible this will also help expedite the processing of the application.
You are required to build or modify the stage I and or stage II equipment according to this application and drawings. Any deviation not previously approved will result in a failure at the time of the pre-burial inspection and will require you to reschedule the inspection at a later date.
